Medication Safety Initiative from Nationwide Children's Hospital

A medication safety initiative has begun at Nationwide Children's Hospital to standardize enteral compound concentrations across the state of Ohio.  This initiative aims to eliminate dosing errors associated with multiple concentrations for compounded drugs.  In order to promote medication safety and provide optimal patient care, community pharmacies can:

  • Ensure compounded medications are accurately filled for the prescribed concentration
  • Providing dosing devices and patient education on dosing volumes when dispensing compounded medications
  • Contact the prescriber or refer to the link below with questions regarding compounded medications

Please use the hospital website to find updated recipes for compounded medications: http://www.nationwidechildrens.org/outpatient-pharmacy-services

Thanks to Alex Swick and Nationwide Children's Hospital for their efforts on this initiative.
